- Two copies of Police FIR Report (in case of a lost passport).
- Two copies of Sworn Affidavits done before a Commissioner for Oaths declaring the passport is lost or mutilated.
- An application letter comprehensively explaining when, where and the circumstances under which the passport got lost or mutilated.
- Colour copy of Kenyan passport (bio data page only).
- Original and color copy of National ID card
- Original and colour copy of birth certificate
- Three passport size photos (2”*2”)
- Colour printout of payment receipt
- A self-addressed envelope with postage stamps to enable the Mission send back the documents.
